As of October 1, 2023 a new benefit plan with Metlife will begin. However, in order to make this transition as smoothly as possible all members are required to enroll using the Employee Navigator portal. (Company Identifier: UFADBA4959) All members must enroll before August 29th.

Beginning October 1, 2023 members basic contributions to the welfare fund will increase from the current $3 per paycheck to $16 per paycheck for individuals and $26 per paycheck for families.

The price of this increase includes the following benefits:

  • $150,000 in life insurance
  • Short Term Disability Coverage
  • Long Term Disability Coverage
  • Critical Illness (cancer, heart attack, stroke)
  • Hospital Indemnity
  • Accidental

During this one time initial open enrollment all members, regardless of your medical history, will be permitted to purchase up to $150,000 additional voluntary life insurance with absolutely no medical questionnaire or exam.

Any voluntary life insurance in excess of $150,000 will require the completion of a medical questionnaire and exam.

Members that previously had additional voluntary life insurance through the union must sign up through this portal to continue such coverage. If the old voluntary life insurance was in excess of $150,000 you will be grandfathered in for purposes of medical questionnaire and exam.

During enrollment, you will also be afforded the opportunity to purchase voluntary life insurance for your spouse and/or children.

As of October 1, 2023 UFADBA will no longer be doing business with AFLAC. Members have the option to continue their AFLAC coverages at the current rate.
